Upgrade 2008 R2 to 2012: Database Engine Services Failed

I am upgrading SQL Server 2008 R2 to 2012.  I received a "Complete with failures" dialog box, which indicated that Database Engine Services and LocalDB failed.

The details for the Database Engine Services states the following:

The name has already been shared
Error code: 0x80004005

The link does not provide any helpful information. I have rebooted and ran Repair several times, with the same results.  The log file is pasted below.  Please help!
